Question 2006: What is the ruling on eye drops, medication, and kohl (surma) for a fasting person?

Imam Baqir, peace be upon him, said: There is no problem with kohl for a fasting person!
2⃣عن محمد بن مسلم عن أبی جعفر فِی الصَّائِمِ یَکْتَحِلُ قَالَ لَا بَأْسَ بِهِ لَیْسَ بِطَعَامٍ وَ لَا شَرَابٍ.

Imam Baqir, peace be upon him, regarding a fasting person who applies kohl, said: There is no issue, it is neither food nor drink!
3⃣عن سماعة بْنِ مِهْرَانَ قال: سَأَلْتُهُ عَنِ الْکُحْلِ لِلصَّائِمِ فَقَالَ إِذَا کَانَ کُحْلًا لَیْسَ فِیهِ مِسْکٌ وَ لَیْسَ لَهُ طَعْمٌ فی الْحَلْقِ فَلَا بَأْسَ بِهِ

Sama’ah says: I asked (the Imam) about kohl for a fasting person; he said: If it is kohl that does not contain musk and has no taste in the throat, there is no problem with it!
4⃣عن أَبِی عَبْداللَّه عَنْ أَبِیهِ فِی حَدِیثٍ أَنَّهُ کَانَ لَا یَرَی بَأْساً بِالْکُحْلِ لِلصَّائِم

In a narration, Imam Baqir, peace be upon him, saw no issue with kohl for a fasting person!
5⃣عن محمد عن أحدهما أَنَّهُ سُئِلَ عَنِ الْمَرْأَةِ تَکْتَحِلُ وَ هِیَ صَائِمَةٌ فَقَالَ إِذَا لَمْ یَکُنْ کُحْلًا تَجِدُ لَهُ طَعْماً فِی حَلْقِهَا فَلَا بَأْس

It was asked of Imam Baqir or Imam Sadiq, peace be upon them, about a fasting woman who applies kohl; he said: If it is not a kohl whose taste she feels in her throat, there is no problem!

Ibn Abi Ya’fur says: I asked Imam Sadiq, peace be upon him, about kohl for a fasting person; the Imam said: There is no problem! It is not edible food!

Imam Sadiq, peace be upon him, said: Kohl for a fasting person has no issue!
8⃣عن الحسین بن أبی غندر قال: قُلْتُ لِأَبِی عَبْداللَّه أَکْتَحِلُ بِکُحْلٍ فِیهِ مِسْکٌ وَ أَنَا صَائِمٌ فَقَالَ لَا بَأْسَ بِهِ.

Husayn says: I said to Imam Sadiq, peace be upon him: Can I apply kohl that contains musk while fasting? He said: There is no problem with it!
9⃣قال الصَّادِقُ : لَا بَأْسَ أَنْ يَكْتَحِلَ الصَّائِمُ بِالصَّبِرِ وَ الْحُضَضِ و بِالْكُحْلِ ما لَمْ يَكُنْ مِسْكاً

Imam Sadiq, peace be upon him, said: There is no issue with a fasting person applying kohl with aloe sap or the extract of the hadhad tree, as long as it does not contain musk!
0⃣1⃣عن جَعْفَرٍ عَنْ أَبِیهِ أَنَّ عَلِیّاً کَانَ لَا یَرَی بَأْساً بِالْکُحْلِ لِلصَّائِمِ إِذَا لَمْ یَجِدْ طَعْمَهُ.

Amir al-Mu’minin, peace be upon him, saw no issue with kohl for a fasting person, provided he does not feel its taste!
1⃣1⃣عن أبی عبداللَّه أَنَّهُ سُئِلَ عَنِ الرَّجُلِ یَکْتَحِلُ و هُوَ صَائِمٌ فقال لَا إِنِّی أَتَخَوَّفُ أَنْ یَدْخُلَ رَأْسَهُ.

It was asked of Imam Sadiq, peace be upon him, about a person who applies kohl while fasting; he said: No! I fear it might enter his head!
2⃣1⃣عن الحسن بن علی قال: سَأَلْتُ أَباالْحَسَنِ عَنِ الصَّائِمِ إِذَا اشْتَکَی عَیْنَهُ یَکْتَحِلُ بِالذَّرُورِ و ما أَشْبَهَهُ أَمْ لَا یَسُوغُ لَهُ ذَلِکَ فقال لَا یَکْتَحِلْ

Hasan ibn Ali says: I asked Imam Abu al-Hasan, peace be upon him, about a fasting person whose eyes become ill—can he apply dry eye medication or something similar, or is this not permissible for him? He said: He should not apply kohl!
3⃣1⃣عن أبی الحسن الرضا قال: سَأَلْتُهُ عَمَّنْ یُصِیبُهُ الرَّمَدُ فی شَهْرِ رَمَضَانَ هَلْ یَذُرُّ عَیْنَهُ بِالنَّهَارِ وَ هُوَ صَائِمٌ قَالَ یَذُرُّهَا إِذَا أَفْطَرَ و لَا یَذُرُّهَا وَ هُوَ صَائِم
📚 Kafi, Vol. 4, p. 114
Sa’d says: I asked Imam Reza, peace be upon him, about someone who suffers from eye pain in the month of Ramadan—can he put medication in his eyes during the day while fasting? He said: He should apply it when he breaks his fast, not while fasting!
✅ From the above narrations, it is observed that applying kohl, eye drops, or medication to the eyes while fasting is permissible as long as its taste is not felt in the throat and it does not contain musk. In cases where the taste is felt in the throat or it contains musk, some narrations discourage it, though a general permission has also been reported.
