Question 875: A person forgets the Qunut until they go into Ruku'. As is their duty, they perform the Qunut after rising from Ruku', then say 'Sami' Allahu liman hamidah', and then 'Takbir'. However, they are not paying attention and instead of going into Sajda, they go back into the Ruku' position, but they remember before reciting the dhikr [meaning, very quickly].

1 عَنْ زُرَارَةَ عَنْ أَبِي جَعْفَرٍ قَالَ: إِذَا اسْتَيْقَنَ أَنَّهُ قَدْ زَادَ فِي الصَّلَاةِ الْمَكْتُوبَةِ رَكْعَةً لَمْ يَعْتَدَّ بِهَا وَ اسْتَقْبَلَ الصَّلَاةَ اسْتِقْبَالًا إِذَا كَانَ قَدِ اسْتَيْقَنَ يَقِيناً.

Imam Baqir (peace be upon him) said: 'When one is certain that they have added an extra Ruku' in an obligatory prayer, they should not count that prayer and should start it from the beginning, provided that they have complete certainty (of the extra Ruku')!'
2 عَنْ مَنْصُورِ بْنِ حَازِمٍ عَنْ أَبِي عَبْدِ اللَّهِ قَالَ: سَأَلْتُهُ عَنْ رَجُلٍ صَلَّى فَذَكَرَ أَنَّهُ زَادَ سَجْدَةً قَالَ لَا يُعِيدُ صَلَاةً مِنْ سَجْدَةٍ وَ يُعِيدُهَا مِنْ رَكْعَةٍ.

Mansur ibn Hazim said: 'I asked Imam Sadiq (peace be upon him) about a person who performs a prayer and then remembers that they have performed an extra Sajda (by mistake); the Imam said: 'The prayer is not to be repeated for one Sajda, but it is to be repeated for one Ruku'!'

Ubayd ibn Zurarah said: 'I asked Imam Sadiq (peace be upon him) about a person who doubts and does not know whether they have performed one or two Sajdas, so they perform another Sajda and then become certain that they have added an extra Sajda; the Imam said: 'No, by Allah! The prayer is not invalidated by adding one Sajda (by mistake),' and he said: 'The prayer is not to be repeated for adding one Sajda (by mistake), but it is to be repeated for adding one Ruku'!'

If someone forgets the Qunut and remembers in Ruku', after rising from Ruku', they perform their Qunut and then go into Sajda.
Now, if they make a mistake and go back into Ruku' after this Qunut, even if they have not recited the dhikr, they have performed a Ruku'. According to the above narrations and others, if someone mistakenly or intentionally adds an extra Ruku' in their prayer, they must repeat their prayer and start from the beginning.
